The Community Collaborative for Health & Wellness (CCHW) began in 2005. It is comprised of representatives from various health and social services organizations or agencies that serve the people of the Bonsall, De Luz, Fallbrook & Rainbow communities. The Health District’s current grant recipients are present and potential grant recipients are welcome to attend as well. The CCHW meets once a month to not only share their programs and lend support to those of other organizations but also to identify, discuss, gain, and/or give information and to initiate or recommend action on matters of concern to the health and well-being of the community.
